People definitely should adjust their grip for FH and BH. Most people do.
As a penholder I always have to switch pressure from thumb to index finger in order to switch from FH to traditional BH.
(actually one of the benefits of RPB is that you don't have to switch grip quite so drastically or at all if you are playing fast).
